Masaji is an eccentric man known for his steadfast commitment to medical freedom, neutral application of care and opposition to Otogakure's medical ethics reforms that ultimately lead to his betrayal of the village. The son of two unremarkable medical ninja, Masaji was groomed from a young age to become a practitioner of medical arts and often shadowed his parents as a young child, desensitizing himself to the stresses of medical care while imbuing in him a sense of duty to patients. As the young boy went through his most important years of development, an unemotional and universal approach to administering medical care made him efficient at assisting doctors but left him detached from events around him, giving Masaji an almost alien capacity for stress and viscera. While this was certainly a benefit when it came to helping those wounded in war, Masaji's inability to rationalize attachments to people left him cold and loyal only to the freedoms that doctors in pre-reform Otogakure enjoyed.
As an adult, Masaji expresses himself through the art of medical ninjutsu and flamboyancy in his fashion, which features layered robes, dresses and painted on ''emotions'' in the form of caricaturized makeup. This streak of non-conformity bleeds into everything he does, from his approach to medicine (which often involves decorative flair or 'alternate approaches to surgery') to his abandonment of Oto as soon as it restricted his freedoms. Now a wanted criminal with a desire to ply his trade, Masaji has conceptualized a group dubbed ''the Medical Science Emancipation Union'' which operates outside the bounds of political alliance or criminal association, administering care to anyone regardless of affiliation or moral standing, so long as they do not harm those who provide them care. To this end, he often travels between countries where Masaji will 'set up shop' in an area, offering medical assistance or other forms of care to any who ask, with his only payment coming in the form of confidentiality. Despite these promises, Masaji has bristled enough patients with his odd disposition that rumors of a ''dress toting medical rebel'' have begun to float around smaller settlements in the major countries.

His primary focus, Masaji utilizes medical ninjutsu in all aspects of his life, utilizing it as his trade and also his primary mode of dealing with conflict. While his healing abilities are on par with the average medical shinobi of this rank, Masaji excels in the use of chakra scalpels due to his experience with complex surgeries and ''guerilla medicine'' from his time on the road, although he hasn't yet mastered the ability to utilize them in combat due to his focus on surgical, willing care. What sets Masaji apart from contemporary medical specialists is his complete disinterest in mastering the titanic strength afforded to masters, viewing brute force as something antithetical to the subtle and beautiful science of medicine, as well as his use of ''non-medical methods'' such as wrapping silk around a wound or using chakra imbued ribbons as tourniquets.

A graceful and deliberate man, Masaji utilizes a free flowing and agility based form of taijutsu based primarily around palm strikes, kicks and holds. Although lacking the structure or additional applications, his approach to taijutsu is often compared to that of the Hyūga's Gentle Fist techniques, with his focus primarily on dispatching a threat in a way that relies on efficiency rather than a satisfying or impressive display of strength. While this dexterity is a boon, Masaji's reliance on dexterity also means that he is not quite as durable as his more hardheaded contemporaries, unable to weather blows to his head or body and simply grit his teeth through it - instead, Masaji practices an almost serpentine method of lowering his physical profile, dodging and bending with a dancer's grace to avoid damage rather than sustain it.

Hand in hand with his agile fighting style and focus on the subtler side of ninja techniques, Masaji is more than capable of practicing the classic feats of ninjas from hiding, to reducing his noise profile and even partakes in tree leaping when a situation gets especially dicey. In terms of focus, however, Masaji excels in the use of disguises and fashion to change his silhouette and make it difficult to associate common features. He will lurch to make himself shorter, wear baggy clothes to appear more rotund and even style his makeup to come across as a woman.

His training as a ninja in Otogakure has given Masaji the capacity to utilize conventional ninja tools, from shuriken to exploding notes, though his preferences lie in the use of senbon both for combat applications also as a form of acupuncture when practicing medical ninjutsu. Although he utilizes what is necessary at the time, Masaji's fascination with senbon has lead to him experimenting with (but not yet succeeding at) the use of senbon as a ''long ranged medical apparatus'' to send short bursts of medically focused chakra into patients. The feasibility of this is as of yet untested, but the steps towards using senbon as vehicles for chakra is perhaps his only exceptional feat when compared to other shurikenjutsu practitioners.
